It's been a while since I've been in fandom, but isn't the joke that he isn't fat but he yo-yo diets anyway? And isn't it usually just Sherlock mocking him for the diets, rather than his weight?

The joke is that Gatiss isn't fat but Arthur Conan Doyle canon Mycroft was, and that Sherlock is an asshat to everyone so he mocks big bro for presumably having been fat.
Too much of Sherlock fandom knows nothing about the original stories and is stupid and shallow so is reduced to making fat jokes about an actor who isn't fat.
One of the many, many reasons I avoid that fandom and stick with canon.
