It does bug me, but as long as they keep mentioning that the scars are there, like in SPN, it feels less like they're ignoring them and more avoiding the two hours in the make-up chair that could be spent actually shooting the show.

From personal experience though, I've had full-on gnarly pieces of metal go through my skin that don't even leave a mark once they are done healing. Ironically, the biggest mark left by anything was scraping up a piece of coral, and not very hard at that, on a snorkeling trip. So for the occasional character, you really can handwave it as just good genetics and fast healing.
