All of them deaths:
Wash from Firefly
Tara from Buffy
Zack from Bones
Fred from Angel
Sirus and Dobby from Harry Potter
Ianto Jones from Torchwood
Oh and both Grey Jr and McSteamy from Grey's Anatomy, who are the two last fictional deaths to break my heart. Also when Dr Yang breaks down.

There are probably loads more, but I can't think of them right now.

I totally forgot to add Fred to HP.

GOOD DAMMIT! I'll just go hide in a corner and cry until I don't have any more tears left...

I always feel oddly guilty that Tara's death didn't bother me as much as I feel it should have. But at the time I still didn't care for Tara, my love for her came later, oddly enough. So after I fell in love with the character, looking back her death was simply awful, but at the time of watching it I was more meh, someone else dying on a Joss show.

Basically every one of these deaths did not bother me at the first watch trough... With the exception of Greys Anatomy.

The hardest part for me is usually watching the next episode knowing the character won't be there. And how the rest of the characters cope with that and so on.

I am an heartless bastard though, so I am pretty sure none of these affected me as much as it seems some people do.

SAYAKA AND KYOKO. I actually find it harder to watch upon subsequent viewings. The first time I watched PMMM, I started crying when it was obvious that Kyoko was going to die with/for Sayaka. The second time around, it was the train scene that set me off, because overhearing that conversation is basically what seals her fate. The third time, I started bawling during Episode 5, when Sayaka is listening to Kamijou play on the rooftop and thinks 'This is the happiest moment of my life.' Not because OH LOL IRONY, but because she's completely sincere: it's the only time when Sayaka doesn't doubt herself, when she truly believes that she's done a selfless and heroic act and seeing other people happy is all she needs. When she starts having second thoughts she's genuinely horrified at her own reaction, and, just, AUUUGGGHHH.

In FFXII the confrontation between Sazh and Vanille reduced me into a big blubbery mess. Sazh especially--his VA did an incredible job. He truly sounded like someone who had lost everything.

ETA: whoops, posted on the wrong comment SORRY ^^;;
