Switching between "he" and "she" for a male character inhabiting a female body feels unnecessary to me, and like it's confusing gender with sex.

Yeah, this.

I once did write a story where the pronoun switched, but that was mostly to hide the fact that the masculine and feminine characters were the same character. (Of course, since the character's own gender identity switched over the course of the story, they'd have needed to change at some point, anyway.)

Yeah, there can be times when a pronoun switch makes sense, like if the character's self-perception changes, or if it's told in the points of view of characters who don't know the feminine character is the same person as the masculine character.
