You can be metabolically healthy but fat, yes. But there's a big difference between healthy and physically able to perform military tasks.
I'm fat and pretty healthy according to my doctor. It isn't as if I can't walk up long flights of steps. I even took a crossfit class last semester. But I can't pass an Army physical fitness test.

Also, what's considered 'fit' in the civilian world is probably not the same as it in within the military. I'm sure people can carry extra weight and still run/lift/kick my ass/whatever in the civilian world perfectly well, but that doesn't mean they would be qualified for X, Y and Z in the service. (On the other side of that, my brother is just shy of being too tall to be a pilot (to his great relief) because cockpits are built a certain size and they just can't safely accommodate someone 6' 5" and above.)
