OP, I feel you.

(And it's much more sensible than my hang up - all my life I've struggled with canons where people just up and go off on magical adventure and suchlike. How are they cleaning their teeth? Where are the toilet facilities? How are character x's legs still smooth and hairless when they've been fighting for their lives against zombies for the past six months??? Yeah...)

Re: leg shaving and such, I thought the way The Hunger Games explicitly talked about that (what with waxing the tributes beforehand and godknowswhat to keep guys from getting beards) was a nice touch. All the excitement of a survivalist drama but with an actual reason for everyone to stay smooth and groomed throughout.

And your hang up is perfectly sensible! How do they maintain hygiene, especially in story types where a modern person is whisked into a proto-medieval environment? It could even be a good moment of character interaction where they have to ask someone what to do!
