I'm actually going to say (and this comes from at least ten years of reading publishers, editors, and agent blogs), don't blame the publishers. The publishers have to sell books (hell, it's quite literally their job), and they can't do that if the book distributors (Barnes and Noble, Amazon, etc) refuse to carry the book because the covers don't fall within certain standards that THEY think will sell.

Of course, that's becoming less of a problem with e-books and self publishing, but for publishers who are selling brick-and-mortar style books, they don't get the final say.

I understand this happens from time to time, but it's a huge exaggeration to imply that the book distributors exercise final approval over every single book that's sold in the stores. It's far more likely that this doesn't apply to the series at all! The bulk of the responsibility for cover art still lies with the publisher.
