The reason I don't have a problem with rape in general in the books is I think the books are all about power, and rape is one of many ways the people in the book use to assert their power over others. Raping women isn't just about power over that one woman, it's asserting the dominance of the man, and of the "victor"--hence the pillaging after a battle. It's like the way children get fostered--i.e., sent out as hostages to other fathers.
So I have no problem with the constant threat of sexual violence to women and children and very often men. But I do think it's pretty glaring that when GRRM creates a cross between a prison and a monastery he not only doesn't make rape a natural part of that world, but there doesn't seem to be much homosexuality going on at all either--what's that about? Yeah, I think we're seeing a blind spot in GRRM here. He has a much rosier (or straighter) view when he imagines a situation like the Wall than he does when he imagines everything else. The Wall's really quite cozy compared to everything else, with Jon making friends there etc.
