In my case it's probably because so many authors cannot for the life of them keep an established couple interesting without falling into out of character dumbass drama. Once the UST ends it's time for "oh god here comes the miscommunication" or "oh god, here comes the unwarranted and never before hinted at insecurity" or "oh god here comes a third party to be sexy and cause shenanigans".

I've seen one fic off the top of my head pull it off. It was Star Trek and pretty much just had Kirk and McCoy keep on going on their normal space adventures only now they share a room. It was pretty awesome.

Yeah, I consider that to be a problem with the author's writing skills rather than the establishment itself. I don't enjoy internal relationship drama precisely because it's formulaic, often out of character, and absolutely boring. And it's everywhere! Like, even in mainstream media, you apparently can't have a couple without bringing in Couple Problems, regardless of whether they make sense or not. It's like you can't have a happy, healthy couple just being awesome and a couple - and that really makes me sad :/ I mean, I write couples because I like them and the idea of them being together, and presumably that's other people's motivation, too, so why would you write them with the intention of making their relationship stressful and miserable? There are so many other things you can do to a couple to show the strength of their bond that don't involve making the audience think they're ultimately incompatible!

+1 that this is a mainstream media problem. It always boggles how people think the problem must be a 'Moonlighting' curse - that once the UST is over, fans will stop caring about the couple. So the cure must be to introduce all sorts of OTHER tension! (House and Cuddy, for a recent irritating-as-fuck example.)
