They're not mutually exclusive - sanity and rationality often goes hand in hand with naivety and close-mindedness. Not always, but often.

Also - you can't pick and choose who fans your work. Yes, he has every right to be disgusted by it, but he has the choice whether or not to use some tact. Unless you're a licensed professional in the mental health field, repeatedly telling people that they need to get help is a jerky thing to do. Fans definitely shouldn't be mentioning the porn to him, but they still have their own right to fan whatever they wanna fan.
