Every death episode from Madoka Magica. Every single one. Especially episode 10 with Mami's meltdown and Madoka's assisted suicide.

SG1 - Heroes from Season 7. If you've watched the show, you know the one.

Also Sybil from Downton Abbey gets an honorable mention because that was probably the best scene from all of season 3.

I cried so hard in the last episode of PMMM and at all their deaths. It was brutal to watch (but I love the series all the same).

Oh god, PMMM. Yeah. If you watch the show unspoiled, as I did, the first one really takes you by surprise. Up until then it's mostly been your standard cute Magical Girl anime, if perhaps a bit more serious in tone than usual, and then WHAM headless Mami, traumatized Madoka & Sayaka.
