Because people tend to change in appearance as they get older, and someone looking normal for 42 would be 'OMG, when was the last time you fucking slept?' at 22.

It's not just about wrinkles. Every part of you starts to sag. I don't understand what you're not understanding here. It's not just your face that changes as you get older. Your metabolism changes, your ability to lose or gain weight changes, you get wrinklier and saggier.

WTF is so difficult about that?

Weight is harder to keep off when you're older. Not for all people, but for many this is true. It isn't ageism or whatever, it's biology and lifestyle changes.

Middle age spread is a thing.

It happens. Even if you aren't gaining weight, you can lose muscle tone which can make you look sorta saggy.
