To amplify the above - not only is the cover design out of the author's hands, it's rarely done by anyone who's actually read the book.

If the author's lucky, the cover artist will be given a rough outline, and will correctly guess how a lot of things should look, or the outline will cover enough significant details that they won't have to guess. If they're really lucky, an actual event from the book will be portrayed without too many wrong details.

If the author's mildly unlucky, the cover artist will be given a rough outline, and will add inappropriate details, or completely skip over something they (or whoever wrote the outline) didn't understand to be significant.

If the author's rather unlucky, they won't be given an outline and will just draw something vaguely appropriate to the genre, maybe inspired by the title. (I think this may be the most common.)

If the author's completely unlucky, the publisher will just grab something from their files and stick it on.
