Blatant OOCness without explanation.

Unrealistic depictions of injuries or illnesses.

High School AU's.

AU's where the characters are a different species ie Transformers or MLP fics having them all as humans. Usually ties in with the above pet peeve.

...I didn't know entire genres could be considered pet peeves? I thought pet peeves were things that just showed up in a fic with no warning that bothered you. How can you be surprised by an AU?

The OP asked for fanfic pet peeves. I hate the idea of high school AU's or AU's that completely changes a character's species.

People like Transformers for the robots.
People like MLP for the ponies.

Take those away, it takes away what makes the show interesting. That's MY opinion. I don't care if people write those, but I won't read them. I can deal with fics where the characters get turned human as long as they turn back later.
