Puella Magi Madoka magica. I cry all the way through, every episode every time I re-watch it. I can't stop re-watching it...
Last Exile, Lucciola, and Alex's deaths. Sometimes I'm fine until Wina lies to Sophia telling her Alex said her name, and then I'm gone.
Aerith's death in FFVII. It makes fighting Jenova afterward a lot more difficult when you are trying to beat her while sobbing pathetically.

If books count I want to add Harry Potter and the Deathly Hallows, The Forest Again. It's also my favorite chapter in the entire series though. I hated the movie version, it just seemed to suck all the emotion out of it.
