If we're talking about the Sims 2:

Actually, that's the one thing there isn't a mod for. You can, however, save right before the birth*, then quit and reload over and over until it's the sex you want.

*This can't be done after the "there's a new family member on the way" message pops up, so do it during the labor pains beforehand.

If we're talking about the Sims 3:

Disregard everything I've just said, because I haven't played that one. Sorry!

Sims 3 actually has a gender choosing function written into the game (not a mod). Feed the pregnant sim apples for a boy, watermelons for a girl.

I've never actually tried for a boy specifically, but I've never had a problem getting girls after feeding up my pregnant sims with heaps of watermelon.
