How do you determine what happens though? I mean assuming there's some sort of PvP or PvNPC conflict, how do you say who "won"? Is it just the DM's call?

Personally I do prefer "no kill" D&D games. There's plenty of other ways to punish overzealous players without wiping out a character they might be overly fond of.

Basically. I think

I get really attached to my characters, especially if I can really flesh them out and get inside of them. My absolute favorite has died, for real, four times. She was saved once by a god call, once by another player taking her place with he god of death and twice by hero points. Thank you Pathfinder!

But all those times (at least a dozen more) where she was an inch away from death are meaningful too, and I don't get any of that with this, thus I don't like it so much. As you can gather from the hundreds of words I'm subjecting you all too.

Yeah mine's been knocked out a couple times, once had his "soul" displaced into a zombie. One memorable TPK they captured us then stripped us of all our gear. Jeebus, was that a pain in the ass for like the 3-4 games where we had to limp back to our hometown.
