It varies. I've had that "wow I can hear and feel my heartbeat and that doesn't sound normal," which usually kicks the anxiety up a notch because it hurts and feels like a heart attack. Sometimes I have to go sit down/put my head between my knees because I feel dizzy and my vision starts to tunnel. But if it's a good day my voice just gets even higher and squeakier than usual and if I'm not supposed to move around a lot, like I'm giving a class presentation and I'll get points off for pacing/hand-wringing, I er--I sort of vibrate. It's weird, and it hasn't happened often because I've only recently got to a point where standing sort of still in front of an audience, and not running off to cry and throw up, was even an option. I actually thought something was going wrong with my vision again, because everything sort of blurred, until I sat down and people started giving feedback. My vision blurred because I was more or less buzzing in place, and it was as weird for other people to watch as it was for me to experience. When the adrenaline wore off I got the usual pounding heart and incipient migraine/urge to puke, but at least I didn't run away screaming and crying. I'm not diagnosed with anything/don't have insurance or meds, but I figure I'm better off than a lot of people because at least I can leave the house.
