I'm not bothered by it on principle, but it can take on weird undertones in fiction (and especially fanfiction, when like A/B is canon, the author's shipping A/C, and B is the one being cheated on).

Almost always.

My favorite, though, is when the canon A/B isn't tagged, so I'll think it's a nice A/C story post A/B-breakup, and the line about A cheating on B is tacked onto the end. IT REALLY PISSES ME OFF.
