Hmm, do you talk a lot about your personal life on tumblr? If you aren't going into extreme detail about things I think you'll be fine. (And even if you do, who's to know if she'll ever look at your tumblr?)

I doubt she would ever be sure of your identity even if she suspected it was you, and if she dropped all contact with you then she probably won't go to the trouble to harass you or anything even if she did find out.

Imo you shouldn't avoid the fandom altogether (though it would be understandable if you did), but if the thought of her finding you worries you that much maybe don't reblog her posts? (Though if her posts are extremely popular she probably wouldn't notice that either.)
