Misogyny aside: Fantasizing about sexing (->being loved) by a monster is basically a very egotistical idea. Because the worse the monster, the better the girl/angel must be who could hope to tame him/make him human.

It's one of the most common fantasies around and it's why "Beauty and the Beast" is a classic. It's why "Jaildate"-services are popular and why Nazis are common fantasy-fodder. It can of course be very unhealthy to hook up with a violent criminal once he gets out.

It's implied that he does stuff to Theon, yes. There's a line about how only Ramsay would see him naked, he walks around with him on a leash, etc.

Considering how he has no problem invading Theon's space as the latter bathes, telling him he can only take his clothes off at his command, kissing him, referring to him as his property, collaring him, castrating him, and involving him in sexual acts, I'd say he most assuredly did with Theon. What's worse is that Theon had to endure this dehumanization every day, rather than being killed afterwards like Ramsay's usual victims.

But other than him, Ramsay's sexual violence is mostly fixated on women, though he's still awful to both genders. He's a psychopath through and through.
