It's not a commission and you're not owed a fill, but on every meme I've ever seen it's considered rude to post a fill that doesn't actually fill the posted prompt. If you post a prompt asking for A/B and someone gives you a story with A/C, that's pretty much universally regarded as a dick move. Some kinkmemes have a "treats" section to post stories that were inspired by prompts, but ended up deviating too much from the original request to actually be considered fills; otherwise, writers can post those fics to their own journals or FFN/AO3 accounts, rather than getting a prompter's hopes up and then giving them something they actively didn't want.

IA. Deviating slightly I can understand, but writing a different pairing than the one requested or going with the exact opposite of what the prompt asks for doesn't make sense to me. The author could post their fic elsewhere saying they were inspired by the prompt, but not post it as an actual fill to the prompt.

I do appreciate when authors ask the OP if the deviation is okay, though. That's a great way to handle it, imo.

Agreed. It's perfectly okay to leave a comment asking the prompter, "Hey, this is a great prompt and I had an idea for a fill, but it's [deviation from prompt]; how do you feel about that?" Then the person who asked for the story to begin with can say either, "That sounds great, I'm looking forward to it!" or "[Suggested change] really isn't my thing, I'm sorry." Filling a prompt is essentially answering a request; if your story doesn't fit the prompt anymore, it's really not that hard to just post it elsewhere, maybe with a note saying where the idea came from.
