This is when people should use author's notes, as a courtesy to their readers. I have one fic that contains just a single brief scene with sexual content (masturbation), but it's too explicit for a "Mature" rating, so I put an author's note at the beginning to inform readers that it wasn't going to have scads of porn, because I didn't want to disappoint people. I don't see why more fic authors can't do likewise -- unless the problem is that they actually fail to understand what the Explicit rating means.
