Do we know that MCU Bruce doesn't have any medical training? It's never said either way so I assume he did.

(Also the joke is that he's not a psychologist.)

Given the investment of time and money that going to medical school requires, it didn't seem reasonable to me to think he had an MD, and it seemed like he was doing a lot more than would be within the purview of someone who had taken an EMT course.

(I got the joke; I just thought it made him kind of a hypocrite since he was willing to practice without qualifications in another context.)

Well he's also pretty much a certified genius, and geniuses in media often have like twenty degrees (even if it's unrealistic). In any case I agree with the below that it seems more like people were asking him for help than him going around offering it.

Given that he definitely has worked with the human body in his studies, even if he doesn't have an MD he's probably still pretty solid in that field's knowledge.

It wasn't that he wasn't willing to practice phsycology, the joke was that he didn't understand why Stark had decided he was the best choice to discuss emotional stuff.

The joke was that he didn't want to listen to Tony and Tony had not even listened to him to begin with.
