The way it worked for me was that I would go to the doctor, explain any issues I have (if it was outside the realm of routine blood test that they perform yearly) and then they would check off the different tests needed. I've had this twice. Once about 6 years ago for one set of hormonal issues and once about two years ago for another set of hormonal issues. One of these blood tests was taken right away and the other one they made me come back after fasting.

I don't know how it works if you are out of the US, but I recommend checking with a doctor and seeing what their policies are.
