Was it later explained in canon that the confusion between whether Watson was shot in the leg or shoulder was due to the fact that he was shot twice? Or is that just the fanon explanation for the inconsistency? (like, is the leg and/or shoulder wound's location brought up more than once?)

All the fics set in the canon 'verse or Granada 'verse or 2009 'verse have him with two separate wounds.

I've only read about 1/4 of the stories so I can't tell.

That's absolutely fanon. The real, cold and drab explanation is that ACD didn't particularly care about the details and simply forgot them, and said different things at different times. But that's so boring, and it's much more fun to say that of course Watson couldn't have been confused about where he was shot, and to come up with intricate explanations for how it all happened.

I assumed it was a mistake, but does that mean ACD never clarified or fixed his own mistake in his stories?

That's correct. He probably didn't even notice them, and certainly didn't care about them. Which is fine.

It is a fanon explanation for ACD's adorable inability to get it straight.

My favorite explanation is that he was actually shot in the behind and wrote down leg/shoulder to spare his Victorian readers.

Doyle just forgot or didn't care. The amusing part is that he says "shoulder" in A Study in Scarlet and "leg" in The Sign of the Four which was THE VERY NEXT STORY.
