Everyone, all the time.

Wilson and Chase on House, Martha and Mels on Doctor Who, Catherine Sakai and Susan Ivanova on Babylon 5... I could go on and on. That said, I'm faceblind and I do that so frequently with people in my own life that having it happen with television characters is hardly surprising.

I don't confuse shows very often, but before I started watching Babylon 5 I could have sworn it was 'the one with the Cylons' (ie, Battlestar Galactica).

A few, although the only one that's coming to mind right now is Owen Wilson and his twice-broken nose.

Characters are a bit easier for me than actors since the pool of possibilities is smaller and they're more likely to have distinctive wardrobes, but in either case I'm often more likely to recognize someone by their voice than their face.
