I've never worn them myself (I'm 5'11") but I used to crossplay with a number of women who did, and apparently the tallest ones are around 3" (beyond that they're too obvious) and the norm is about 2". The built-in ones can be quite comfortable, apparently, (though these are much more expensive) but the actual inserts (as a separate piece and not part of the shoe's design) can be uncomfortable and are especially hard to run in. They lift the heel primarily, putting as much pressure on the toes as, well, conventional heels do.

You can wear them in conjunction with any kind of shoe, really - at least my friends did. Doubling them up with a men's shoe or boot that already had a slight heel or thick sole for an extra boost was pretty common, but you can also put them in running shoes and the like.
