It's not rude at all. On the contrary, it's very helpful to make a list so you don't forget anything while you're there.

Requesting tests is also not frowned upon unless you don't have a real reason for the tests to be run. Personally, I would say (and I'm sure most physicians would agree with me) that not having a full workup done in seven years is a good enough reason for you to request all the blood work you need.
