the title is DRAMAtical Murder. With Murder. And DRAMA in all caps. It's meant to be a pretty disturbing R18 game. and it is R18, so... the kids playing it are doing it at their own risk.

(and the "normal" "good endings" are not violent at all, for what it's worth. they are generally sweet yaoi fluff. it's the bad endings for choosing all the bad options that are insane.)
