Just look at it this way, OP. The Hobbit is told from a different POV than LOTR, so Bilbo just saw his younger self as Martin Freeman but he really looked like Ian Holm with a scotch tape face lift.

Don't know if anyone is aware of this, but this is literally the case. He had his face scotch taped back for this scene. They mention it in the EC.
