Then if it is such a rare alternative use, it's likely that a lot of people don't know what it means or that it's misogynistic/racist.

I wouldn't really call it rare exactly. I've heard it fairly often irl, on the internet and even in songs. People who use it as slang know what the implications are since the very definition is something that is "ghetto" or "trashy" and is usually applied towards women.
