He is mauled by a mother bear protecting her cubs. She crushes him, throws him around, and flays the skin and meat off his back, down to bone. IRL the guy, Hugh Glass, had to roll in maggots to keep his back from necrotizing while he crawled for six weeks through the frozen mountains, refusing to die like a pioneer-age John McClane.

And then, apparently, when he reached some kind of civilization (a Native American tribe, I think?) they sewed a hide to his back to protect it while it healed.
