I once killed someone. It was completely accidental, and it was mainly because of my inexperience with equipment and not being supervised properly, but I still killed someone and escaped blame-detection for it. It was in an old-folks home and the death just written off as age related, but it was still because of me. I quit working there when I found out what happened, I shouldn't been left in that position with my level of inexperience and it makes me wonder how many other age-related deaths are due to fuck ups or lack of supervision.

I'm so sorry. Although if they said it was age related... unless it was a massive cover up, it sounds to me like it might have been partly that they were already ill and elderly.

It was more like no investigation whatsoever. It was taken for granted that all deaths were age related.

Now, I don't know exactly what transpired but... It's really, really hard to ascribe blame when someone is ill enough to need mechanical life support, OP. If this is what I think it is, and it was a palliative care situation, then you might have been doing them a favour in the long run.

Age-related deaths aren't "due to fuck ups". Age-related deaths are inevitable. It's so very difficult in end of life care to say that someone would definitively have survived longer with a quality of life they found pleasant and worthwhile if things had been different. Most of my experiences have found the opposite to be true.

Again, I don't know the specifics. I'm just saying there's a chance you shouldn't really be beating yourself up for this.

Seconding - I used to work for a law firm that represented a lot of nursing homes, and if anything was even slightly off the family would have sued for wrongful death/negligence. The fact that the mistake apparently escaped detection makes me think there was nothing to detect.

I'm very sorry. That must be hard on your conscience. But know that if you were not supervised properly when you should have been, that's a failing of the system, not your personal failing. Sadly you now live with the consequences, but their death was a result of several fuck-ups, not just yours.
