The description comes from the line of a character who met Watson again for the first time after he returned to England.

"Whatever have you been doing with yourself, Watson?" he asked in undisguised wonder, as we rattled through the crowded London streets. "You are as thin as a lath and as brown as a nut."

Implying that he had NOT been "thin as a lath and brown as a nut" lest time they had met, meaning before Watson served in Afghanistan. So yeah, this is definitely about Watson having tanned. Surprisingly enough, white people tan. Even Brits.
