There's cultural connections with danger, sex, etc. that people's minds automatically associate with smoking. There's also a big emphasis brought to both the fingers and the mouth. Being able to visibly see someone inhale and exhale the smoke is intimate. Plus, taking a drag hollows out the cheeks, like sucking a dick.
I... uh... spent a lot of time watching hot dudes smoke as a teenager.

It's not an exact thing, it's just a lot of individual elements that come together to make something hot. Like... lingerie. Or uniforms. Just taken on their own, there's nothing sexy about them. But you smash them together with a lot of cultural/social concepts and then stick them in a sexual context and suddenly you have something that a lot of people think is sexy.
