I don't think it's either one, it's something that gets tacked on before one's birth name and used only by people who you're very close to - your parents and older relatives, your siblings, etc. In this case, Wei Wuxian (who knew the character in question when the character was a little boy) calls him A-Yuan, but his contemporaries do not.

And Jiang Yanli calls her brother "Ah Cheng" and anyone else who even tries that would probably get their legs broken, hahaha.
