Not sure I'd go as far as calling people cretins, but point taken. I've been guilty of the former, and subjected to the latter. Having been on "both" sides, there really is no excuse. Even if you generally have a hard time with these types of interactions, or struggle with guilt and/or an anxiety, you should never screw over the other person. In the former situation, speak up before it's too late so that someone else has the chance to step in - it's better than nothing. In the latter, simply saying "thanks!" really is the bare minimum.
