It's different for every fic. Sometimes it's a line from the fic, sometimes it's a play on a line said in canon (but never an actual line said in canon), sometimes it's just a vague phrase or word that I think fits it somehow. I never use song lyrics because I try to keep my fic titles to something that someone else hasn't already used (preferably at all, but at least within the fandom).

Pretty much this, for me. Sometimes I'll use a line of poetry, especially if the poem it comes from was an epigraph for the fic.
