As someone who does occasional streaming: it's because time flies when you're having fun playing games so what seems like thirty minutes might be three hours and you should REALLY stretch and drink water in that time. A reminder that time has indeed passed is nice. And if you're doing a marathon stream of twelve hours for, say, charity but still having fun, someone asking if you've remembered to eat is nice when you're focused on delivering the content. It's basically, "Remember that the perception of the passage of time when you're streaming may be off and reminding someone to hydrate and maybe pop a snack every once in awhile can remind the streamer that more time has passed than they think." But that's a lot longer compared to the much shorter message that most people get that is, "Remind your streamer to eat and drink water." Everyone who hangs in streams knows why. You, the watcher, are likely going to have a much more accurate perception of time compared to the streamer.
