Gotham's implied to have a lot of low income families/people, so a lot of them probably *can't* leave.

In "No Man's Land" it's mentioned at least once that a lot of the people who stayed behind despite the city being disowned by the US stayed because they couldn't afford to leave.

And the wealthier people would live in the safer areas and might not feel the need to move.
