I always thought it was about abuse, but I'm most familiar with Jeff Buckley's version, which is rather dark sounding. It's possible that how someone interprets it could just be up to whatever version a person hears, as there are so many.

Cohen's original version is also fairly dark, although probably more about fucked-up, tense, negative relationships more than "abuse" as stuff (like about 60% of Cohen's songs)

But yeah, I think there's been a lot of interpretations over the course of the song becoming a standard as it has become, and so there's a lot of angles to take on it, which is perfectly fine
