This reminds me of a time travel fic where Cas is talking to the current-time version of himself who was wearing Claire. Cas called Clairestiel "Sister" and she called him "brother". By the end of that chapter Clairestiel decided the only reasonable way of dealing with the situation was for her to merge with Cas. Later when Cas was talking to his Jimmy (yes, he still had his own vessel's soul with him), he mentioned his younger self had changed gender view and considered himself male now.
