I don't understand at what time they replace doctors. does something have to happen in canon for a new doctor to generate or whatever? or is there just surprise! a new one every once in a while?

Usually the old Doctor has to die. So far he's had radiation poisoning a couple of times, actual poisoning, grevious bodily harm from TARDIS crashes or big falls or exploratory heart surgery, forced execution by his own people (technically forced regeneraton, but to him largely the same), absorbing energy from the time vortex (which possibly comes under 'radiation poisoning', now that I think of it, making that probably his #1 COD) and what was possibly old age (Hartnell/One).

Other Time Lords have also regenerated in a few ways. It's implied via Romana that it can be optional (as in, you don't need to be fatally injured), and if you do it optionally you get more control over the next body. Various plots involving the Master also implied that you can be given more regenerations or have them taken away, as well as being forced into them a-la the Doctor's second regeneration (though, really, killing him will do). This sort of all went up in the air with Nu!Who and the loss of the Time Lords, so it's possible that there aren't any limitations to how many you can have anymore.

But, um. Yeah. Essentially it happens when the Doctor is mortally wounded/killed.

I haven't heard about the exploratory heart surgery before, which one died of that?

Seven into Eight, in the movie with Paul McGann. He got shot first, mind, but it was non-fatal until some human went rooting around in his chest to try and figure out if he had two hearts or if that was an x-ray double-exposure. He woke up, went into seizures, flatlined. As you do, really.

I thought One died due to radiation poisoning as well. Yeah, he was getting old, but the radiation exacerbated it.

Could have been, I haven't seen One in a while. Huh. That really is his number one cause of death, then. Seriously, Doctor, start investing in hazmat and radiation suits.
