Is this normal?

[personal profile] ansela_jonla 2014-03-29 09:57 pm (UTC)(link)
I managed to get a dentist appointment. What no one told me is that if you have a regular NHS dentist that you are signed up with, they have a legal obligation to give you an emergency appointment within 24 hours of you calling. You just have to make it clear that it's an emergency. Even if it's just a "turn up and sit in the waiting room until the dentist has a free moment to see you", they have to give it to you!

So now I have antibiotics again. For the fifth time since last August, I have amoxicillin sitting in my medicine drawer, next to my lansoprazole. And once again it's making me absolutely sleepy and forcing me to sleep in the middle of the day. I was expecting this, so that's not a worry.

What is worrying, however, is my gum. On Thursday, when I went to the dentist, it wasn't all that swollen and the pain was more of a low ache in the gum/jaw (at least until she poked it with a pointy thing, at which point displeasure was expressed). Two days later it is now swollen up to the point that closing my mouth is painful, and even the softest solid foods are out of the picture.

I wasn't given any painkillers, only an antibiotic prescription, and OTC painkillers have stopped having any effect (I need to not use them for a few months now). I still have my appointment for Thursday (just need to check the time of it), and she said she'd give me metronidazole if the amoxicillin hasn't worked by then.

Amoxicillin should not make you sleepy. I am on it right now for an URI. And I have taken it before since I was a kid. Sleepiness has never been a symptom. Neither has swelling. ESPECIALLY swelling.

I think you are having an allergic reaction to the medicine. I would call if I were you RIGHT NOW. Do not wait.

What's your dosage, by the way?

Amoxicillin has always made me sleepy, every single time I've had to take it.

It's a 500g tablet, one three times a day, for 5 days.

That's not a unusual dose. But I'd still call because that's an usual symptom.

Tiredness is a rare, but known, side-effect of amoxicillin. It's one of those things where you're supposed to let your doctor know the first time it happens in order to rule out a more serious complication, but then just be aware of it going forward. My brother has managed to freak out everyone who has ever lived with him by having all the rare side-effects to certain antibiotics, which have included everything from tiredness to paranoia and twitching.

...I'm much like your brother, only add in that I am allergic to all but one class of antibiotics. Fortunately I managed to narrowly avoid the reaction where your skin melts off and you end up like a third-degree burn victim. (Sulfa drugs. Never again.)

Medications affect different people in different ways. While there are common symptoms for stuff, saying "such and such shouldn't make you sleepy because it never makes me sleepy" doesn't make any sense. It's like saying "pizza shouldn't give you heartburn because it never gives me heartburn".

Unless the fatigue is extreme, like the patient is falling asleep where they stand, then no, it's not something to panic about and it's not unusual. When people have unusual allergic reactions to medications, it tends to go waaaaaaay beyond feeling sleepy.

Seriously, I'm starting to wonder if some people are getting their medical information from fanfic or Scientiologists or something. Sleepiness and localized swelling at the site of an infection are not symptoms of an allergic reaction.

I don't know your full story so apologies if you get asked this all the time:

Is there any chance that you have something stuck in there? My gums usually swell if a bit of tooth floss comes off and I find that basicly squeezing until it comes out works. It saved me £60 on an emergency appointment when I discovered that.

Otherwise, I might recommend grabbing another emergency appointment. That is clearly affecting you a lot.

It's a wisdom tooth issue, one of them has decided to get stuck halfway out and it's creating a pathway for my gums to get infected. The dentist started the paperwork for referral to a different clinic that can remove it surgically, but that's going to take time.

I dread finding a job before then. The sort of stuff I'm applying for would not pay enough to cover dental surgery.

Oh wow.
Best of luck!

Not a doctor, but an infection can cause swelling, especially if your gum got irritated by being poked with the pointy thing. It's only been a couple of days on the antibiotic, but if the swelling doesn't ease up over the weekend, I'd try to get another emergency appointment.

My advice is to call NHS direct right now. As in tonight.

See your dentist again tomorrow, and get another emergency appointment if the swelling is worse.

If that's not possible, check for emergency dental clinics at your nearest hospital. Or go to your doctor as an emergency patient. You need to get this sorted out as soon as possible. You don't want to leave it until the swelling impedes your breathing.

It's true that antibiotics can take 3-4 days to work, but after 4 days of not eating you'll be pretty weak.
