After seeing a lot of tweets like this, I gave up on even trying to listen to the podcast.

"We found it one day in the sink" turns the "heart" into a literal heart, which is the primary joke. The secondary joke is that a sink is kind of a strange place to find the heart (you'd think it would have been hidden or something). The tertiary joke is that if you reread the first sentence, you hear the emphasis on the words differently, like "Ohhhh, HOME is where the heart is! I was wondering where that heart went!"
The last sentence is just a weird creepy random thing.

The original phrase means "Home is the place where you feel like you belong", but in Night Vale it means there is an actual heart in your home. And that it hums at night.
