It does not refer to male characters who show emotions.

(Meanwhile, the female love interests are the ones getting endless amounts of shit for showing emotion and upsetting the precious man.)

In most of those cases a lot of those male characters in particular tend to have Much Angst and Sad Sobstories, or are even anti-heroes/villains, and are woobified up the wazoo. Hence, manpain.
The vampire diaries fandom and show is a perfect example of this.
