That being said, it's fairly realistic for multilingual people to occasionally throw in a foreign word.

I find that a more common slippage is between two languages when I'm fluent in neither one, and sometimes then I don't even notice code-switching. Or I'll say a word, and not remember which language it's in.
The challenge really is making it seem unconscious, imo, which is precisely what they fail to do in most cases.

It also I think depends on the character and how it is used. Kurt from the X-men, yes he speaks English, but if he's startled he say something in German. Jean-Paul and Jeanne-Marie from Alpha Flight will talk to their team in English, but occasionally use French with each other. In some context and for some character it makes sense. Antonio from Power Rangers Samurai would occasionally use Spanish words and while I can see how to some it might have been anyone, the character was generally so enthusiastic about everything that he just came off as adorable.

But sometimes it does just feel wrong/weird, I mean... look, we've all seen cringeworthy 'fangirl Japanese' in fics (substitute whatever language, depending on fandom I guess), and it can come across as really unnatural, especially if the word choice is just a bit off.
Mostly I can deal with it even when it feels awkward, though-- it only really bothers me if I'm supposed to be in a fantasy universe where I'm accepting that everyone is speaking English purely for the audience's benefit, and that in their world, they're speaking their own language, and I'm all immersed and then someone speaks some really glaring bit of another earth language. I mean, it depends on how it's done and what the universe is, whether there is a connection to our world or not, but that's the kind of thing that will make me stop reading.
