I would honestly not write anything. Shady as shit tbh. Even if you're QUITTING, why do they need to suddenly ask your "gender", "marital status" or "sexual orientation"? You won't need to be on their files and you shouldn't be on their files after you leave. Filling out that kind of unnecessary info seems sus.

AYRT - Agreed. As for marital status it's standard practice and I kind of get it, because married people have to show their marriage certificate at admission so maybe they're supposed to show that when leaving too, alongside every other document (but I wouldn't know the details seeing as I have been single all the while). But yeah, suddenly asking for info that isn't in any official documents is pretty shady I agree.
