If you're like "I hate/dislike/am afraid of gay people but whatever, they can do what they want, I won't stop them, they can be on my tv shows and my neighbours and that, but... they're *so* going to hell/They are *so* gross and disgusting and perverted/they really creep me out" then yes, that's not tolerance. Not that I'm saying you think that, that's just an example.

Well you shouldn't get them leaping to your defense. You CAN write black characters and hate black people as well, but that doesn't make you less racist, same as you can write gay characters and still be homophobic and hate them in real life. And that's not a good thing.
No one would suddenly start saying "Oh I see what she's saying here, she's saying that she hates black men walking around wearing lots of bling and joining gangs! I hate that too!!!" and people saying "gosh can't she express a hatred without everyone jumping on them! There's apparently no middle ground!" and talking about how they 'tolerate' black people but they're no racist...

Exactly.
I do think there's a difference between being 'slightly uneasy' around gay people and saying that you 'hate' gay people though. There's a big difference. No one's saying that if you 'tolerate' gay people you have to join gay pride parades or watch two guys kissing right in front of you. But if you say you 'tolerate' them and that's clearly... not... what is going on, then... argh. Yes, that's this elusive middle ground of prejudice people are constantly claiming they're in the middle of.
Basically, I don't like the word 'tolerance'. It implies that you don't like it but let it go, which is still against it. If I say 'My dog tolerates a baby pulling on his ears' it doesn't mean he likes it, supports it, doesn't hate it, or anything, all it means is that he doesn't bite the baby or complain about it through growling.

oh, gotta add though, religious tolerance I can understand as a phrase. Ie that you're one religion/belief system so you can't be in more than one really, and you don't mind the others being around or whatever. That's fine. But when people use 'tolerance' for something like race or sexuality, that's when it really pisses me off.
